Help senior patients with referrals, appointments, authorizations, condition management and education by calling and engaging them on a monthly basis and document conversations and update charts. Help senior patients with:

Must Have: 

1. EMR experience

2. Basic excel/sheets knowledge

3. Decent typing skills.
